The Otsuka Welfare Clinic was established in Peshawar, Pakistan by Otsuka Pharmaceutical in 2003 with the assistance of 23 Otsuka group companies that operate in Asia Pacific and the Middle East. The companies collaborated to open this clinic in order to provide free medical services to Afghan refugees who had crossed the border into Pakistan. The clinic provided support to more than 880,000 people during its 14.5 years of operation up until its closure in December 2017.

Since 2001, more than 1.8 million refugees have fled to Pakistan from neighboring Afghanistan. The refugees in the border town of Peshawar are desperately short of food and clothing. At the time the clinic was founded, Otsuka Pakistan Ltd., an Otsuka Pharmaceutical affiliate, was providing intravenous solutions to the area through the Red Cross, and as a company doing business in the region, Otsuka was determined to provide direct assistance to refugees. An in-depth study indicated that a free medical clinic was needed to provide support to people in refugee camps who lacked adequate medical treatment, and the Otsuka Welfare Clinic was established in 2003.

The clinic employed nine regular staff, including three physicians, assisted by nurses, pharmacists, and medical assistants. The clinic saw mainly women and children, providing treatment to approximately 300 patients a day for respiratory, digestive, dermatological, obstetric, gynecological, ophthalmological and otolaryngological ailments. In addition to helping Afghan refugees, the clinic provided care for local residents. In all, the clinic treated more than 880,000 people since it opened.

In July 2010, heavy flooding caused by severe rainfall in the region caused considerable damage at the nearby Aza Khel Refugee Camp. Clinic staff traveled to the area to set up a temporary treatment facility to provide care for people.

In December 2011, Japan's ambassador to Pakistan recognized the Otsuka Welfare Clinic for its outstanding contribution to community welfare through the provision of free medical care over the prior eight years, and for having a positive influence on Japan-Pakistan relations.

Subsequently, in August 2014, Otsuka Pharmaceutical was awarded a Japanese Minister of Foreign Affairs Commendation in recognition of the company's contribution over many years to medical care for Afghan refugees in Pakistan and local residents of the area.

As the return of Afghan refugees to their home country continued to gain momentum in 2016, and a continuation toward stability was anticipated in 2017, it was decided to bring the program to an end by year-end 2017.
